The short answer
Choose Thailand if you live in Australia or New Zealand, the 9–12-hour nonstop flight matters more than the price, you can stay 10–14 days, and you have verified the surgeon on the Medical Council of Thailand register rather than trusting the package.
Choose Mexico if you want the surgeon named in writing, an accredited hospital, hotel nights and 12 months of follow-up included in one price, and that price to be about half of Bangkok’s. From the U.S. and Canada it is a 1–5-hour trip; from Sydney, Melbourne, Brisbane or Auckland you fly nonstop to Los Angeles and connect to San Diego, Puerto Vallarta or Guadalajara.

Price is the last thing to compare, not the first. But here the numbers are not close: a Bangkok sleeve is AU$10,090 for the hospital alone, before 6–8 hotel nights, and ALO’s all-inclusive sleeve is about AU$6,300. Compare the package, then the total, then decide.

What the price really covers
Thailand is cheap for Australians compared with a private sleeve at home (AU$15,000–$20,000 without cover, or AU$3,500–$7,500 out of pocket after a 12-month waiting period on Gold hospital cover). It is not cheap by international standards. Bangkok’s big private hospital groups price for the global market, and most sleeve packages cover the hospital only: you add 6–8 hotel nights, meals and a 10–14-day stay because the clinic wants to see you before you fly. New Zealanders face the same maths against private quotes of NZ$19,000–$27,000 and public waits of 12–24 months.
Mexico is the lowest all-in price of the three, and ALO’s $4,500 buys the tier that costs extra elsewhere: Dr. López operates personally in every case, in an accredited private hospital 30 minutes from a U.S. airport, with English-speaking staff, 2 hotel nights and 12 months of follow-up by his own team. Extras (≈ $650) counted the same on both sides: optional complication insurance (≈ $285), first-year vitamins (≈ $300) and the patient’s meals. Safety: what the Australian Government says, and how to verify any clinic
Smartraveller, the Australian Government’s travel advice service, has a page on going overseas for a medical procedure. Its advice is worth quoting: “Many hospitals overseas are a similar standard to hospitals in Australia. But quality and standards in some countries can be poor.” It asks you to “check if the surgeon who will do your procedure is qualified and accredited” on an official website, to “compare the complication risks they give you to those mentioned in Australia” and be wary of much lower figures, to take someone with you, and it reminds you that “Medicare doesn’t cover you when you’re overseas” and that “basic travel insurance policies rarely cover medical tourism.” That is good advice for Thailand, for Mexico and for anywhere else.
None of this means Thai surgery is unsafe. Bangkok has JCI-accredited hospitals and bariatric teams with excellent results. The pattern in the cases that go wrong is the same one we see in Mexico: patients chose by brochure, did not know who would operate, and had no one responsible for them once they flew home. Whatever country you pick, these are the questions that separate a good decision from a gamble:
- Who exactly will perform my surgery, and can I speak to that surgeon before I pay?
- How many sleeves did that surgeon do last year, and what is the leak rate?
- Which hospital, which accreditation body, and is there an adult ICU on site?
- Is the surgeon on the Medical Council of Thailand register, or on the U.S. and IFSO registries for Mexico?
- What happens at 2 a.m. on my third night if I have pain or a fever?
- Who follows me after I fly home, for how long, and how do I reach them?
- Is the quote in writing, including hotel nights and the extra days, and what is the policy if a revision is needed?
- Does my travel or health insurance cover complications of this procedure, at the clinic and back home?
