Why Fundoplications Can Fail Over Time

A Nissen or Toupet fundoplication wraps the upper stomach around the lower esophagus to recreate the antireflux barrier. How the Single-Stage Conversion Works

The procedure has three coordinated phases performed laparoscopically in one operation:

  • Fundoplication takedown — Dr. Alejandro López carefully releases the prior wrap from the lower esophagus, preserving the diaphragmatic attachments.
  • Hiatal hernia repair (when needed) — Any recurrent hiatal hernia is repaired before reconstruction.
  • Roux-en-Y gastric bypass — A small gastric pouch is created and connected to a limb of the small intestine, completing the bypass.

Because the bypass anatomy diverts acid downstream and the small pouch produces very little acid, reflux is controlled by the new anatomy itself — no wrap or fundoplication required.
